Appointments are prioritized on the basis of medical urgency and availability. In most cases, appointments typically require your doctor's referral. Many insurance plans require a referral from a plan doctor, so please check with your insurance company to determine what, if any, requirements it has for care at CIVA.

Office hours are 9 am to 5 pm, Monday through Friday.(Except on standard holidays.)

After normal business hours this line will be answered by the CIVA Answering Service, who will contact the CIVA physician on call. This is also the appropriate phone number to contact a CIVA physician or staff member during business hours.
